Death certificate of Janina Krolak
This is a typical "full" certificate (nonetheless called "skrocony" - "short") issued in the times the Communist Polish People's Republic (1945-1989).  After liberation of Poland in 1989, the Polish emblem changed to the traditional crowned eagle and the official name of Poland changed to Rzeczpospolita Polska (Republic of Poland).  It could be hand-written or typed.
